Holy shit! Can you blame him/her for NOT cutting it off? That'd be a crime against nature.
If you want to get technical about it, they don't cut it off per se--they basically hollow it out, turn it inside out, and make a vagina with it; IIRC, the head of the penis is usually used to help form a clitoris. Ironically enough, considering how complicated female anatomy can be, it's easier to make a surgical MTF than a FTM, because the penis is a dual-function body part, and it's always harder to add something than to take it away. (Yes, I know a MTF--why do you ask? :-)
