The actor who goes back and forth with another actor in being the biggest of the big historically has had so much work done to his face the past month he can barely make any kind of facial expression other than some kind of half smile/grimace.

I think they are referring to box office which is usually Harrison Ford or Samual L Jackson. I am guessing this is Ford.

the only metric for the kind of "big" that would find him back-and-forth neck-in-neck with another actor is their box office tally. famous roles, talent and popularity are too subjective to say that anyone is the current "biggest". cruise's face is looking very puffy and stiff, so i think it's him.

hm, i thought that tom cruise was the biggest at the box office, but he's not. going to guess harrison ford.
Once upon a time, the two big, comparable actors that went back and forth with Oscars and nominations was Pacino and DeNiro. Would have to see recent photos, cause I have no idea if they've went under the knife.
Tom Hanks gets my vote. Tommy is filming right now, I'm thinking that the movie execs wouldn't want him to screw up his ability to emote. Such as it is. Harrison Ford doesn't strike me as the type to care what he looks like.

I think it's Stallone who's had the work done and it's Arnold who he has been in neck and neck competition with for years.
