cocaine's a diuretic because it constricts blood vessels (so fluid in the body is peed out.) as with other drugs, after awhile the body loses it's ability to function normally...the blood vessel elasticity and responsivenes is messed up and you get a bloated look. this goes away when the person detoxes, just as opiate users regain their ability to poop normally in time.
Like with Lohan, you get skinny (not always but mostly), it is just your face that looks fat.
