I feel like this is someone youngish to early middle age because he's worried about future roles. Cult movies are typically fantasy/SciFi, horror, and stoner flicks. Lots of actors have been in those. I don't think the "Pirates" movies would be considered cult movies. They were hugely, hugely successful. Cult movies are usually modest hits at best. So, not Depp. Hmmm.
I'd say Jeff Bridges has been in tons of cult movies, the cultiest being "The Big Lebowski". I'd also say he is A list. He's a little past early middle age, but he's still young enough that he could be worried about future roles. I hope it isn't him, because I really like him.

When I think of cult movies I think of Steve Buscemi. I don't know if he's A list (he is in my book).

My first thought after reading this went to Keanu Reeves (not as A List as he once was but we all know who he is :p ) Then reading the rest of your comments Johnny Depp made more sense.
RJ, JD has been in lots of "cult" classics (Nightmare on Elm Street, Crybaby, Platoon, Edward Scissorhands, Ed Wood, Whats Eating Gilbert Grape etc most of which didnt have huge success at the box office but are considered "cult") so saying not to count him because of those "pirate" movies is a bit silly lol REALLY hoping hits not him hes the Dave Grohl of the film world hehe
Agree with RJ that cult films tend to be sci-fi, horror, stoner flix. And Jeff Bridges has been in several cult-type films (Big Lebowski, Tron series). Could also be Woody Harrelson (Natural Born Killers, Zombieland) or Keanu Reeves (Matrix series, Constantine and Bill & Ted's Excellent Adventure).
Johnny Depp seems possible. In the clip of his cameo in Life's too Short that's out he is very funny but he seemed very off. Like Michael Jackson-ish and not in the way RG & SM exaggerate the celebrities. The first thing I noticed was how different he seemed.

Don't think he will be able to keep it secret. If it's affecting his work, then that will get around plenty quick enough.
Discobitch, true. Depp has been in some major cult movies. "Edward Scissorhands" and "Ed Wood" are every bit as culty as some of Jeff Bridges' movies. I just have a hard time believing that a star as big as Depp would be afraid on not getting work. It seems like he turns down three roles for every one he does. Plus, he and Tim Burton are very, very close. The only way Tim wouldn't hire him for a picture is if Depp were too ill to work. Depp has always struck me as a man who would be more likely to walk away from acting all together than to be one to worry about losing roles.
Johnny Depp was my first thought as well. Pretty much all he did for many years were indies and now-cult films like 'Ed Wood', 'Cry Baby', 'Dead Man', etc, etc, etc. He doesn't look well these days, but it could be due to his heavy drinking.
What about James Franco?
Sounds like Bill Murray to me, sadly.
I don't think this blind gives us sufficient details to start throwing out random names of actors who may have a "health condition".
That being said, my favouritest guess is Cathy's. Travolta's male pattern baldness ftw!
When I think cult favorite movie actor, I think John Cusack. Haven't seen much of him lately which seems to fit this blind.
